Current Patient FAQs

  • For prescription refills, have your pharmacy fax a medication refill request to (317) 818-8933.

    The Nurse Practitioner may require an appointment before prescribing a refill, depending on how long it has been since your last appointment. Our office will contact you if an appointment is needed.

    It may take up to 72 hours for a request to be processed. Please plan to ensure there is no lapse in your medication.

  • Please download, print, and bring the completed forms with you to your appointment.

    If you do not have Adobe Reader, please click the Get Adobe Reader button to download it.

    If you have questions, please call the office at 317-580-9330 Ext. 100 or text us at 317-955-9447 (TEXT ONLY #).

  • As of 6/29/22, please bring your insurance card to EACH visit to be checked by our staff. We will need to reschedule your appointment if your insurance card is unavailable when you check in.
