We use this time to thoroughly review your health history, hear your story, and identify what other tests and analyses are required to provide the necessary data to give us the most complete picture of your health.

It is our time to listen, and our goal is to provide you with the highest level of personalized care possible. We are committed to partnering with you to achieve optimum health.

To get the full picture of your health, our New Patient intake process requires that your complete four forms:

• Patient Registration

• Health History

• HIPPA

• Medication/Supplement List

Below, please find printable PDF versions for each of the New Patient forms. Please download, print, and bring the completed forms with you to your first appointment.

Please arrive 15 minutes before your appointment to complete our registration process.

You will need your insurance card to complete the registration process. We will need to reschedule your appointment if your insurance card is unavailable when you check in.

Below is a list of what you need to bring to the appointment.

• Photo ID

• Insurance Card

• Completed Patient Registration Form

• Completed Health History Form

• Completed HIPPA Form

• Completed Medication/ Supplement Log
